The Department of Pediatrics and School of Public Health are seeking a Case Manager to work with our grant-funded program to develop a hospital-community violence intervention program. This person will work with the team to develop and work with patients who are victims of firearm violence (ages 18-30).  The Case Manager will work with a multi-disciplinary team and community partners to address firearm injury within our community. We are a passionate and skilled group that is seeking someone who is interested in community-based research and interventions.  In addition, they will provide professional case management and social work services and oversee the care of patients.  Provide indirect services to patients: provides utilization review oversight of the treatment program; acts as an advocate for the patient/family with the treatment team and third party payers.

Position Location:

Schedule: Full-Time, Mon-Fr, (Flexibility to work a hybrid schedule)

Position Key Accountabilities:

1.  Oversees patients’ care and progress in treatment in order to prevent both the under-utilization and over-utilization of inpatient treatment services through feedback to and from the patient, significant others, the treatment team, third party payers and community resources. 
2.  Provides third party payers with clinical data in order to justify, advocate for, and arrange for necessary services for the patient while in the hospital, as well as after discharge. 
3.  Works with a multidisciplinary treatment team to develop an individualized treatment and discharge plan that enables patients to quickly begin or return to outpatient treatment programs; 
4.  Develops a discharge plan that will support patient's ongoing recovery and development. 
5.  Assesses patients at admission and throughout the treatment program; conducts interviews with patients and significant others to determine assets as well as challenges to optimal patient functioning both while in the program and upon discharge to a lesser restrictive level of care. 
6.  Establishes and maintains relationships with community resources to assist patient’s needs at discharge for ongoing treatment, residence, financial support, transportation, education, and vocational rehabilitation. 
7.  Provides direct services to patients which includes the following: assessment, treatment and discharge planning; individual, couple, and family education, crisis prevention and supportive counseling.
8.  Must be able to respond to emergency situations. 
9.  May be assigned to act as mentor to new employees and to social work students. 
10.  Other duties as assigned.

Certification/Skills:

Licensed Master Social Worker (LMSW) from the Texas Board of Social Work examiners required, certification in case management preferred.

Minimum Education:

Master’s degree in social work from a school approved by the Council on Social Work Education.

Minimum Experience:

Five years experience in medical social work, to include two years in case management.

Physical Requirements:

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.

Residency Requirement:

Employees must permanently reside and work in the State of Texas.
